Opportunity Overview

We are developing a Centre of Excellence in older adult care at Hampstead Centre. This presents an exciting opportunity to be part of a dynamic multidisciplinary team that will help shape and implement an innovative model of care, setting new standards in patient outcomes.

This initiative aligns with our strategic vision for delivering world-class healthcare. Hampstead Centre is launching the CO-ACT program, which focuses on providing exceptional care for older adults transitioning from acute to ongoing care.

As an integral member of our CO-ACT multidisciplinary team, you will play a crucial role in improving the health system and making a real difference in the community.

Job Responsibilities

  • Manage and oversee patient treatment within the service, working closely with Consultants including Geriatricians and General Practitioners, Nurses, and Allied Health professionals.
  • Engage in Continuous Quality Improvement initiatives and participate in ongoing educational activities to enhance expertise and that of colleagues.
  • Gain experience in multidisciplinary care models, geriatric care, and engagement with the South Australian Civil and Administrative Tribunal.
  • Develop crucial conversations skills with patients and substitute decision makers.
