Family Reunification Specialist

The Family Caring 4 Family program is a pioneering initiative that identifies kin placement pathways for children and young people in out-of-home care. The goal is to reunify families under safe and supportive conditions.
Key Responsibilities- Engage clients through home-based visits, turning initial agreements into ongoing consent and interest in becoming a Kin Carer.
- Identify potential kin carers and establish relationships with children and broader family members.
- Facilitate Family Link Decision Making meetings, ensuring family consensus on Placement Plans to reunite children with family care arrangements.
- Support assessments of carers, including initial screening, Blue Card submission, and PAC evaluations.
- Deliver case-managed services according to the program model of care, maintaining minimum case load requirements for equitable access to services.
You'll be part of a unique and supportive culture where every team member is valued and respected as an individual.
Requirements- Bachelor's degree in a relevant field with at least 4 years' experience in case management, family support, child protection, youth support, or related fields.
- Diploma in a relevant field with at least 6 years' experience in case management, family support, child protection, youth support, or related fields.
- At least 6 years' experience in case management, family support, child protection, youth housing, or related fields, with a commitment to completing a Diploma while working.
