The role we are looking to fill is Family Caring 4 Family Program Coordinator. This position will be responsible for identifying kin placement pathways for children and young people residing in the child safety residential OOHC system.

The goal of this program is to re-unify family under safe and thriving conditions.

Our program provides coordinated and culturally safe support and interventions to families, aiming to build family capacity, strengthen community connections, and safeguard the wellbeing of children.

This involves assertive kin finding, holistic case management, and collaboration with multiple service providers.

Families are empowered to navigate challenges, develop essential life skills, and access therapeutic support in efforts of family reunification.

Key Responsibilities include:

  • Effectively engage clients through cold-call home-based visits, turning initial 'kin agreement' home visit into ongoing consent and interest in becoming a Kin Carer.
  • Facilitate FLDM meetings with broader family kin, gaining family consensus on a Placement Plan to reunite child with family care arrangements.
  • Assist in the assessment of carers, including initial screening, Blue Card submission, and PAC assessments.
  • Maintain accurate and detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.

Requirements for the position include:

  • Bachelor's degree in a relevant social services field with at least 4 years case management experience in family support, child protection, youth support, housing or related field.
  • Diploma in relevant Community/Services field with at least 6 years in a case management role in family support, child protection, youth support, housing or related field.
  • At least 6 years case management experience in family support, child protection, youth housing or related field. A commitment to complete Diploma (funded by ATSICHS) while working.
  • OR non-indigenous and having worked for an Aboriginal Community Controlled Organisation for at least 4 years.
