Job Description

We are seeking a skilled Community Care Specialist to join our team. As a key member of our community support team, you will provide essential services to individuals with disabilities, including those receiving support through the National Disability Insurance Scheme (NDIS) and Traffic Accidents Commission (TAC).

Your primary responsibilities will include:

  • Delivering individualized support plans to enable clients to maintain and increase their independence.
  • Providing Part-Time and Casual opportunities for flexible working arrangements.
  • Working across various locations within the West Wimmera region.

To succeed in this role, you will require:

Essential Requirements
  • Effective communication and organizational skills to deliver quality support services.
  • A valid NDIS Worker Screening Check and Working With Children Check, plus an annual police check if working on the TAC program.
  • A commitment to principles of empowerment, inclusion, choice, and control when supporting individuals maximize their independence and participate in community life.
  • A current First Aid Certificate Level 2.
  • A Victorian Drivers Licence.
Desirable Qualifications
  • A qualification in Aged Care, Disability, Individual Support, Community Services, or an equivalent field.
  • Previous experience working in community or residential aged care or disability settings.
Key Responsibilities

As a Community Care Specialist, your key responsibilities will include:

  • Providing support services for individuals receiving community-based support, including NDIS participants and community-aged care clients.
  • Assisting clients to achieve their individual goals outlined in their Support Plan or Care Plan.
  • Promoting positive community attitudes towards disability, ageing, and inclusiveness.
