About This Role
We are seeking a dedicated Support Professional who is passionate about supporting individuals with sensory impairments and additional disabilities. The successful candidate will work under the Active Support model, providing person-centered support to enable clients to enhance their independence, inclusion, and well-being.
Key Responsibilities
- Provide daily support to clients using a person-centered approach under the Active Support model
- Empower clients to achieve their goals and improve their quality of life by recognizing and responding to their needs and preferences
- Ensure client notes are up-to-date and relevant care plans are implemented
- Collaborate with key stakeholders such as families, guardians, and other service providers to ensure seamless communication and coordination of care
- Work collaboratively as part of a dedicated team to deliver high-quality services that meet the needs of clients
- Adhere to all policies and procedures, including those related to health and safety, confidentiality, and reporting incidents
Requirements
- Minimum Cert III in Disability and/or related discipline (currently undertaking studies or completed) OR a minimum 6 months experience in a similar field
- Ability to address triggers and redirect behaviors in a safe and respectful manner
- Flexible and empathetic attitude, with a willingness to learn and adapt to new situations
- Current First Aid and CPR Certificate (with at least 3 months until expiry)
- Current NDIS Workers Screening Check (or willing to obtain one)
- Current Working with Children Check (or willing to obtain one)
- Valid driver's license and access to a reliable vehicle
- Proof of COVID-19 Vaccination and Booster Vaccination
- Available for regular shifts on a 24/7 roster (including sleepover and active nights)

- First Aid Certification (preferred)
