Occupational Therapist Job Opportunity
">- A dynamic Occupational Therapist role available in a challenging and rewarding environment.
This position combines Occupational Therapy and Care Coordination. As an Occupational Therapist, you will provide assessment, intervention, and management to promote functional recovery, self-management, and empowerment for clients with safety, functional, medical, and environmental issues impacting independence. Your work will involve home assessments, equipment prescriptions, falls prevention, pressure care, functional assessments, and activities of daily living retraining.
">Your Key Responsibilities- Provide person-centred care coordination for high-risk clients with chronic diseases, comorbidities, and complex aged care needs.
- Conduct holistic assessments, coordinate health and social care, monitor goals, facilitate interventions, and ensure linkages with hospital and community services to optimize safety and wellbeing.
To be successful in this role, you will require:
">- Bachelor of Occupational Therapy
- Minimum 3 years' experience in relevant acute/community settings
- Strong assessment and case management skills
- Excellent communication and collaboration skills
- Knowledge of community supports available for clients
