Job Opportunity: Dementia Support Coach

This role is an excellent opportunity for professional growth and exposure to a new project. We are seeking a highly skilled individual to fill the position of Dementia Support Coach in our dementia centre initiative.

Job Description:

The successful candidate will facilitate on-ground support, implement relationship-based support strategies, model and coach clients, carers and care staff. Key responsibilities include:

  • Collaborating with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand individuals living with dementia and provide best practice support and care.
  • Implementing recommendation plans on the ground, drawing on modelling and coaching techniques, and seeking guidance as required.
  • Facilitating and supporting the trial and implementation of brokerage items with clients and their carers, and seeking guidance as required.

Key qualifications and skills required for this role include:

  • Strong organisational abilities, effective time management and ability to prioritise in a busy work environment and competing demands.
  • Ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure consistent provision of exceptional service to internal and external customers.
  • A current driver's licence and safe driving record.
  • Willingness to travel and work intrastate and interstate to support delivery of services.
  • Qualification in a relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar.
