We are seeking a highly skilled and compassionate Registered Nurse to join our virtual team in an Advanced Care Planning and Palliative Care role. As a key member of our team, you will provide professional care advice to elderly residents and their families, utilizing a person-centred framework to deliver best practice care and outcomes.
Key Responsibilities- Provide advanced care planning and palliative care services to residents and their families
- Utilize a person-centred framework to deliver best practice care and outcomes
- Mentor and lead the broader care team to ensure high-quality outcomes
- Ensure compliance with accreditation standards in Aged Care
- Collaborate with multidisciplinary teams to deliver comprehensive care services
- Current APHRA registration as a Registered Nurse
- Postgraduate education in Palliative Care
- Previous experience in an Advanced Care Planning and Palliative Care role
- Ability to coach and support other team members with best practice outcomes
- Advanced clinical knowledge of aged-related and end-of-life health issues
- Mature and highly empathetic approach, with the ability to relate and converse with older people and their families/loved ones
