About the Role

We are seeking a highly skilled Registered Nurse to join our virtual care team in an Advanced Care Planning and Palliative Care role. Reporting directly to the iAgeHealth Manager, you will be an integral part of supporting our residents and their families to make informed decisions around their future healthcare needs and desires.

As a key member of our multidisciplinary team, you will provide best practice care and advice to elderly residents, provide mentoring and leadership to the broader care team, and ensure that high-quality outcomes are always delivered.

This role will see you providing all aspects of palliative care, from symptom and pain management to advanced care planning. Your compassionate approach and ability to have open and honest communication with all stakeholders will ensure that our residents in rural and remote areas receive the best quality of life they deserve.

About You
  • Current APHRA registration as a Registered Nurse
  • Postgraduate education in Palliative Care
  • Previous experience in an Advanced Care Planning and Palliative Care role
  • Ability to coach and support other team members with best practice outcomes
  • Advanced clinical knowledge of aged-related and end-of-life health issues
  • Mature and highly empathetic approach, with the ability to relate and converse with older people and their families/loved ones
  • Commitment to working as part of the multidisciplinary team
  • Demonstrated ability to seek out and effectively utilize new knowledge as it relates to nursing practices
  • Computer literate and the ability to learn new procedures and programs in alignment with the iAgeHealth Platform
  • Knowledge and understanding of the accreditation standards in Aged Care
  • Commitment to embrace, model, and work within the core values of iAgeHealth (McLean Care)
