Advanced Care Planning and Palliative Care Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our virtual team in an Advanced Care Planning and Palliative Care role. As a key member of our multidisciplinary team, you will provide best practice care and advice to elderly residents, utilizing a person-centred framework to ensure high-quality outcomes.
Key Responsibilities- Provide advanced care planning and palliative care services to residents in rural and remote areas
- Utilize diagnostic tools and live data to inform clinical decisions
- Collaborate with the care team to ensure seamless delivery of services
- Provide mentoring and leadership to the broader care team
- Ensure compliance with accreditation standards and ACFI documentation requirements
- Current APHRA registration as a Registered Nurse
- Postgraduate education in Palliative Care
- Previous experience in an Advanced Care Planning and Palliative Care role
- Ability to coach and support other team members with best practice outcomes
- Advanced clinical knowledge of aged-related and end-of-life health issues
