Key Responsibilities- Provide advanced care planning and palliative care services to residents in rural and remote areas
- Collaborate with the care team to ensure high-quality outcomes and effective communication
- Develop and implement care plans that meet the unique needs of each resident
- Provide coaching and mentoring to other team members to ensure best practice outcomes
- Stay up-to-date with the latest clinical knowledge and research in aged-related and end-of-life health issues
- Current APHRA registration as a Registered Nurse
- Postgraduate education in Palliative Care
- Previous experience in an Advanced Care Planning and Palliative Care role
- Ability to work effectively in a virtual team environment
- Strong communication and interpersonal skills
