About the Role

As a dedicated service professional, you will be responsible for delivering high-quality services to clients and carers across designated regions.

The role focuses on the Western and Geelong region, with occasional involvement in Ballarat as needed. You will be based at one of our Retirement Villages in Melbourne's West, such as Point Cook or Bacchus Marsh.

Key Responsibilities

  • Conducting comprehensive assessments and developing tailored care plans
  • Providing advice on service options and coordinating referrals
  • Empowering clients and carers to manage their care plans with confidence
  • Maintaining accurate client records and ensuring compliance with funding and legislative requirements
  • Monitoring service delivery and responding to changing client needs

What We Are Looking For

To be successful in this role, you will require:

  • Tertiary qualification in Health or a related field (nursing or allied health)
  • Experience in case management and care planning
  • Strong understanding of aged care reforms and consumer-directed care
  • Excellent communication, time management, and networking skills
  • A current Victorian driver's licence
  • Openness to travel across Western Victoria, Geelong, and Ballarat
