At BaptistCare, you will assist elderly individuals in maintaining their well-being within the comfort of their own residences. If you are passionate about empowering individuals to live independently and enhance their quality of life, this role may be ideal for you.

As a Community Care Specialist serving clients in the Orange & Bathurst regions, you will support clients and their families in accessing services that enable them to remain in their homes. You will facilitate individualized care planning and goal-setting centered on our well-being philosophy.

Key Responsibilities:

Conduct home visits to clients in the community who require assistance Engage in the development, execution, and assessment of services and programs tailored to clients' unique needs Build relationships with families and caregivers that recognize their requirements Collaborate with clients to establish realistic and attainable individual goals Implement best practices and evidence-based care approaches while working closely with a multidisciplinary team Maintain documentation and utilize client record management systems This position will provide support to clients across the LGA's of Orange and Bathurst

Qualifications and Skills:

Candidates should be comfortable working independently in the community, possess excellent verbal communication abilities, and demonstrate an empathetic, patient, and caring demeanor.

Required qualifications include:

Bachelor's Degree or Diploma in Community Services (Service Coordination), or Diploma in Community Services (Case Management), or equivalent knowledge and experience A valid driver's license Intermediate computer proficiency Strong time management and organizational skills Proven experience in navigating funding constraints and achieving service delivery goals within government program guidelines Experience in aged care or relevant transferable skills will be highly regarded

A company vehicle is optional, and you will be provided with a mobile phone and laptop.
