As a Community Care Coordinator serving clients in the designated areas, you will assist clients and their families in accessing services that enable them to continue living at home. You will facilitate personalized care planning and goal-setting focused on enhancing well-being.
Key Responsibilities:
Conduct home visits to clients in the community who require assistance Engage in the design, execution, and assessment of services and programs tailored to meet clients' unique needs Build relationships with families and caregivers, recognizing their needs Collaborate with clients to establish realistic and achievable personal goals Implement best practices and evidence-based care approaches while working closely with a multidisciplinary team Maintain documentation and utilize client record management systems Provide support to clients across the specified local government areasQualifications and Skills:
To excel in this role, you should be comfortable working independently in the community, possess strong verbal communication skills, and demonstrate an empathetic, patient, and caring demeanor.
