As a Community Care Specialist serving clients in the designated areas, your role will involve supporting clients and their families in accessing services that enable them to continue living at home. Key Responsibilities:
Conduct home visits to clients in the community who require assistance Engage in the creation, execution, and assessment of services and programs tailored to meet clients' unique needs Build relationships with families and caregivers, recognizing their requirements Collaborate with clients to establish realistic and attainable individual goals Implement best practices and evidence-based care strategies, working closely with a multidisciplinary team Maintain documentation and utilize client record management systems This position will support clients across the designated local government areasQualifications and Skills:
Candidates should be comfortable working independently in the community, possess excellent verbal communication skills, and demonstrate an empathetic, patient, and caring demeanor.
Required qualifications include:
