About the Role

Are you passionate about delivering high-quality care to aged clients through their transition to care programs? We are seeking a dedicated and experienced Aged Care Coordinator - Transition Care Expert to join our team in Townsville.

Key Responsibilities
  • Lead and collaborate with interdisciplinary teams to ensure effective implementation of program objectives, ensuring seamless transitions for our clients.
  • Provide guidance and supervision to program staff to maintain the highest standards of care and service delivery.
  • Develop and implement comprehensive therapeutic care plans for clients, considering their unique needs and circumstances.
  • Conduct consultations with community services to enhance collaboration and support networks for clients.
  • Maintain accurate and up-to-date documentation and clinical records in accordance with regulatory standards.
  • Facilitate smooth and well-coordinated discharge planning in collaboration with healthcare professionals, ensuring continuity of care for clients post-transition.
  • Provide support to clients and their families to address the emotional aspects of the transition period.
  • Delve education sessions to clients and families, promoting health literacy and empowering them to actively participate in their care.
About You
  • You will have excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders to achieve positive outcomes.
  • A strong understanding of therapeutic care planning and discharge planning processes is essential.
  • You will be able to work effectively in a multidisciplinary team, prioritizing tasks and managing time efficiently.
  • Excellent written and verbal communication skills are a must, with the ability to maintain accurate and detailed records.
