Lead Care Coordinator - Home Care Services

We are seeking a skilled and compassionate individual to join our team as a Lead Care Coordinator for our Home Care Services.

  • About the Role:
  • This is an exciting opportunity to make a real difference in the lives of vulnerable people across Australia.
  • You will be responsible for managing home care package budgets, providing regular reviews with clients and their families, and ensuring compliance with community care best practice, privacy and legal obligations.
  • You will also provide advocacy and support for clients and people living with dementia and/or behaviours of concern, coordinate stakeholders and external services to enhance care for clients, and promote the service within the catchment area.
  • In this varied role, you will have the opportunity to work closely with a dedicated team to deliver high-quality care services that meet the needs of our clients.

Your Skills and Qualifications:

  • Certificate III in Individual Support (Home and Community) or equivalent
  • Applied knowledge of current aged care legislation, funding mechanisms, government policies, and statutory regulations
  • Minimum one year experience in leading a service delivery team and planning, co-ordinating and implementing care services in the home care/community care environment
  • Ability to use mobile devices to record and manage information
  • Intermediate computer skills with the ability to use Word, Excel, Outlook and PowerPoint
