About the role*
Due to continued growth, we currently have an exciting opportunity for an enthusiastic and professional individual to join our Home Care Coordination team at LHI Retirement Services. The role of Home Care Coordinator is to support clients to remain living at home through the promotion of independence and choice. By employing professional skill and knowledge, the Home Care Coordinator will assist clients to develop attainable goals and, in turn, coordinate a range of services that support and promote these goals.

Having a flexible approach to ensure appropriate coverage for care, which includes a rotating on call roster.

About this role, you will be: *
- In collaboration with the Client Services Manager respond to and meet prospective clients and their families to understand their care needs.
- Develop client-centred service plans and individualized budgets.
- Monitor, review and evaluate services provided to clients.
- Provide clinical care to clients as required.
- Managing incoming requests in a friendly and professional manner.

**Requirements**:

- Minimum - Certificate 4 or Diploma in Community services or relevant discipline or equivalent experience
- Highly Desired
- Relevant diploma qualifications will be highly regarded.
- Relevant experience in the coordination of Home Care Packages

**Skills / Experience / Knowledge**:

- 3 years Community Aged Care is essential
- Experience in Case Management for Home Care Packages
- Current Drivers Licence
- Sensitivity to the needs of older people of different social, economic and cultural backgrounds
- Ability to work as part of a team
- Ability to be self-directed and work autonomously
- Well-developed interpersonal and communication skills.
- Effective time management and problem solving skills
- Proven ability to prioritise work, manage pressure and deal with competing demands effectively.
