Home Care Manager

An exciting opportunity has become available for an experienced Home Care Manager to join the team on a permanent full-time basis. As a member of the leadership team and reporting to the Director of Care, this role will see you leading the effective delivery of the St Louis Home Care Packages that supports our clients with making informed choices and resulting in a better-quality of life by staying as involved as possible whilst living in their own homes.

**About the Role**:

- Oversee the Home Care team to ensure the successful delivery of individually tailored packages and support services to clients who fall within the eligibility criteria of the Home Care Packages Program.
- Work with the St Louis leadership team with the transition to the Support at Home Program
- Asses of applicants for acceptance onto the program in accordance with HCP / SAH & CDC Guidelines and St Louis policy and procedures and allocation to appropriate Coordinators.
- Ensure that the client group remains consistent with HCP/ SAH & CDC target group requirements.
- Promote and represent St Louis for the delivery of Home Care Services and Community Programs
- Liaise with relevant industry groups as appropriate to enhance the effectiveness and efficiency of service delivery
- Planning and oversight of the scheduling of support workers and third-party suppliers to deliver quality care and services
- Build a high-performance team through timely recruitment and effective performance management of staff.
- Oversee the development and review of Care Plans in consultation with clients and their families

**To be successful in this role you will have**:

- Previous experience in driving high performance and managing community program performance.
- Knowledge of Aged Care legislation, specifically in home care programs and working toward the support at home program
- Social, Home & Community Services, Aged Care, Disability, Health Industry or related relevant experience
- Demonstrated experience in developing and implementing operational plans aligned to the organisational strategic plan.

**Qualifications / Experience**:

- Relevant qualification in health or social sciences.
- Experience with Budgeting and Financial Management - Desirable
- Experience in in the provision of care services in Aged Care
- Experience of delivery of services in the community is essential
