Clinical Care Co-ordinator Position

We are seeking a dedicated and experienced individual to join our team as a Clinical Care Coordinator (CCC). This is an excellent opportunity for a motivated and organized professional who is passionate about delivering high-quality care services.

The successful candidate will be responsible for building strong connections with clients and their families, assessing client needs and resources, conducting Client Care Assessments and Care Planning, and maintaining accurate records. They will also support clients in meeting their goals, provide feedback to Home Care Workers, and promote values that ensure exceptional care delivery.

To succeed in this role, you will need:

  • A current registration certificate for a Registered Nurse.
  • Excellent skills in assessing, documenting, and implementing client's care planning objectives.
  • Experience in assessing and planning care needs.
  • A sound understanding of Aged Care Quality Standards and NDIS Practice Standards.
