Clinical Care Co-ordinator Role

We are seeking a dedicated and compassionate individual to join our organisation as a Clinical Care Coordinator (CCC) on a full-time basis.

  • Key Responsibilities:
  • Establish meaningful connections with clients and their families.
  • Conduct thorough assessments of client needs and resources.
  • Develop and implement comprehensive care plans.
  • Maintain accurate records in accordance with the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework.
  • Provide ongoing support to clients to achieve their goals.
  • Collaborate with Home Care Workers to ensure seamless delivery of services.
  • Prioritise tasks efficiently to meet the demands of a fast-paced environment.
  • Foster a culture of excellence by promoting the values of the organisation and implementing contemporary approaches to care.
Qualifications and Skills
  • A current registration certificate for a Registered Nurse is essential.
  • Excellent assessment, documentation, and implementation skills.
  • Experience in assessing and planning care needs is highly desirable.
  • Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework.
  • Strong organisational, administrative, and time management skills.
  • Ability to work effectively in a team setting.
  • Well-developed problem-solving, interpersonal, written, and communication skills.
  • The successful candidate must have the right to work in Australia.

Essential Requirements:

  • National Police Check.
  • Driver's Licence.
  • NDIS Worker Screening Check.
  • First Aid & CPR Certificate.
Preferred Qualifications
  • AHPRA Registered Nurse registration with previous leadership experience.
  • Up-to-date working knowledge and experience achieving accreditation standards.
  • Ability to identify and improve areas of clinical risk and quality guidelines.
