Clinical Care Coordinator

We are seeking a dedicated and highly motivated individual to fill the role of Clinical Care Coordinator. As a key member of our care team, you will be responsible for building strong connections with clients and their families, assessing client needs and resources, and conducting comprehensive care assessments and developing personalized care plans.

The successful candidate will have excellent skills in assessing, documenting, and implementing client's care planning objectives, as well as experience in assessing and planning care needs. They will also possess a sound understanding of aged care quality standards and NDIS practice standards, and be able to work in a fast-paced environment.

  • Key responsibilities include:
  • Building strong relationships with clients and their families
  • Assessing client needs and resources for health, welfare, and other facilities and services
  • Conducting comprehensive care assessments and developing personalized care plans
  • Maintaining accurate records as per aged care quality standards and NDIS practice standards
  • Supporting clients to achieve their goals
  • Providing feedback and support to care workers

The ideal candidate will have a current registration certificate for a registered nurse, strong organizational and administrative skills, and the ability to work effectively in a team setting. They will also be required to obtain a national police check, driver's license, NDIS worker screening check, and first aid and CPR certification.
