About the RoleWe are seeking experienced Social Workers to join our multi-disciplinary team in acute hospital settings. As a Social Worker, you will be responsible for assessing patient needs, developing care plans, and coordinating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities- Liaise with health professionals, family members, and community agencies to develop comprehensive care plans
- Conduct psychosocial assessments to identify patient needs and formulate effective discharge plans
- Provide advanced social work services, including counseling, case management, and education
- Maintain and manage complex caseloads within clinical units and social work departments
- Eligibility for membership of the Australian Association of Social Workers
- A Bachelor of Social Work or recognized equivalent
- Experience in an acute hospital setting preferred
- The ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities
- Knowledge of working within an electronic medical record
