Job Title: Clinical Social Worker - Acute Setting

We are seeking a highly skilled and dedicated Clinical Social Worker to join our team in an acute setting. This is an exciting opportunity for an experienced social worker to work with medical oncology, surgical and general medicine patients, from a diverse adult age group.

Responsibilities:

  1. Support patients and their families around adjustment and crisis intervention.
  2. Address complex social issues impacting on recovery, including providing counseling services.
  3. Showcase skills in working with people from a range of diverse backgrounds.
  4. Complete complex psycho-social assessments and develop patient centred care plans.

This role requires the ability to participate in on-call rosters and work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios.

Requirements:

  1. Demonstrated experience in providing social work in acute and subacute settings.
  2. Knowledge and experience working closely with inter-professional teams.
  3. Proven clinical experience enabling you to bring knowledge of healthcare reforms and initiatives.
