Key Role: Comprehensive Assessor
About the PositionWe are seeking a skilled and compassionate Comprehensive Assessor to join our team. This is an exceptional opportunity to make a meaningful difference in the lives of aged clients.
The successful candidate will be responsible for conducting thorough assessments of aged clients, developing care plans that promote restoration of function and maximise independence, and reviewing support plans to facilitate referrals or service coordination.
Key Responsibilities- Conduct comprehensive assessments of aged clients and develop individualised care plans
- Review support plans and coordinate referrals or services for older people
- Maintain effective working relationships with community providers and stakeholders
- Formal qualifications in Nursing or Allied Health
- Valid Victorian Driver's License
- Excellent clinical assessment skills
- Strong typing and computer literacy skills
- Effective verbal and written communication skills
- Ability to work independently and as part of a collaborative team
- Previous experience in ACAS/aged care assessment
This role requires full vaccination against Covid-19 and adherence to our organisational immunisation policy.
