About the Role

This role is an excellent opportunity to make a meaningful difference in your community as a Dementia Care Specialist.

You will be responsible for providing personalized case management that supports dignity, choice, and wellbeing for people living with dementia. This may involve home visits, group settings, and educational talks to increase understanding of dementia within the community.

In addition to delivering high-quality care, you will also conduct timely assessments and care plans to ensure services are optimized and effective. Your strong communication, presentation, and advocacy skills will enable you to build and maintain strong relationships with clients, families, carers, and stakeholders.

To be successful in this role, you will require a Diploma in Community Services, Dementia Care, or equivalent qualification, along with experience working with older people, particularly those living with dementia.
