We're seeking a compassionate and skilled Dementia Care Adviser to join our team. As a key member of our community, you'll be responsible for providing person-centred case management that supports dignity, choice, and wellbeing.

The ideal candidate will have a strong understanding of dementia, its progression, and its impacts on individuals and carers. You'll need excellent communication, presentation, and advocacy skills to deliver evidence-based advice and advocacy during home visits and group settings.

  • Responsibilities:
  • Provide person-centred case management that supports dignity, choice, and wellbeing
  • Deliver evidence-based advice and advocacy during home visits and group settings
  • Conduct educational talks to increase understanding of dementia in the community
  • Complete timely assessments and care plans to ensure services are optimised and effective
  • Build and maintain strong relationships with clients, families, carers, and stakeholders
  • Manage budgets, maintain accurate client records, and ensure compliance with organisational standards

Requirements:

  • Diploma in Community Services, Dementia Care, or equivalent qualification
  • Strong understanding of dementia, its progression, and its impacts on individuals and carers
  • Experience working with older people, particularly those living with dementia
  • Excellent communication, presentation, and advocacy skills
  • Proficiency in MS Office and client database systems
  • Current C Class driver's licence and access to a reliable vehicle
