Key Responsibilities
  • Deliver person-centred case management to support dignity, choice, and wellbeing.
  • Provide evidence-based advice and advocacy during home visits and group settings.
  • Conduct educational talks to increase understanding of dementia in the community.
  • Complete timely assessments and care plans to ensure services are optimised and effective.
  • Build and maintain strong relationships with clients, families, carers, and stakeholders.
QualificationsWhat You'll Bring
  • Diploma in Community Services or equivalent qualification.
  • Strong understanding of dementia, its progression, and impacts on individuals and carers.
  • Experience working with older people, particularly those living with dementia.
  • Excellent communication, presentation, and advocacy skills.
  • Proficiency in MS Office and client database systems.
  • Current C Class driver's licence and access to a reliable vehicle.
Ideally, You'll Also Have
  • Experience in facilitating community education programs.
  • Knowledge of aged care service systems and funding arrangements.
