
Family Support Coordinator

This vital position involves coordinating care for children in out-of-home care, focusing on permanency, cultural connection and healing.
Key responsibilities include developing and reviewing case plans, supporting family time, building relationships with birth families and extended kin, and working towards guardianship outcomes where appropriate.
As a Support Worker, you will play a critical role in ensuring the safety and well-being of children in care, while also providing support to their families.
Responsibilities:- Developing and reviewing individualised case plans that meet the unique needs of each child.
- Building strong, respectful relationships with birth families and extended kin, while also supporting family time and reunification efforts.
- Collaborating with clinical teams to develop behaviour support plans that promote positive outcomes for children.
- Identifying and assessing risk factors, and developing strategies to mitigate these risks and ensure child safety.
- Working closely with other professionals to ensure a coordinated approach to supporting children and their families.
