Job Description:

We are seeking a RN Health Coach to provide high-quality patient care and support through remote communication tools. The ideal candidate will have strong clinical experience, excellent communication skills, and the ability to work independently.

Key Responsibilities:
  • Provide remote patient coaching and education using digital tools.
  • Offer telephone triage for patients with acute symptoms.
  • Collaborate with patients and caregivers to increase engagement and overcome healthcare barriers.
  • Guide patients through changing medical therapies and connect them with clinicians as needed.
  • Serve as a liaison between patients and the healthcare system, including providers, clinics, and pharmacies.
Requirements:
  • Current active Registered Nurse license in a compact state.
  • At least 2 years of clinical experience in a specialty practice or acute care setting.
  • Bachelor's degree required.
  • Highly developed written and verbal communication skills.
  • Analytical skills to problem-solve patient issues and prepare reports.
