About the Role:
  • We are seeking a dedicated professional to fill a permanent full-time position at St Vincent's Hospital.
  • This role will involve providing social work interventions as part of a collaborative team delivering person-centred, holistic end-of-life care.
Main Responsibilities:
  • The successful candidate will participate in our 24-hour on-call crisis service offering emotional and practical support to patients and their families during critical situations.
  • Psychosocial and risk assessments will be undertaken to address the needs of patients and their families.
  • Care coordination and end-of-life planning will be supported by working closely with medical, nursing, and allied health staff.
  • Therapeutic counselling, advocacy, and support will be provided to patients, families, and carers experiencing grief, loss, or distress.
Key Qualifications:
  • A degree in Social Work or a related field is required.
  • Experience in palliative care or a related field is highly desirable.
  • Excellent communication and interpersonal skills are essential for this role.
