We are seeking an experienced Care Coordinator to support clients in achieving independence and continuing living at home.

About The Role

This key role delivers evidence-based interventions under Aged Care Funding to help clients regain independence.

  • A Degree in Occupational Therapy with current registration.
  • Proven ability to assess occupational therapy needs and design tailored programs for aged care clients.
  • Postgraduate experience working across diverse aged care services, including community-based settings.
  • Thorough knowledge of health and well-being frameworks, including reablement and restorative care models.
  • Demonstrated capability to operate effectively both as part of a multidisciplinary team and independently.
  • Exceptional written and verbal communication skills.
  • Proficient in computer literacy, with a willingness to utilise required software to facilitate remote work.
  • Commitment to upholding values through professional interactions.
  • Possess a valid first aid certificate or demonstrate a willingness to obtain one.
  • Possess a valid driver's licence and access to a reliable, registered vehicle with minimum Third-Party Property Damage Insurance.
