We are seeking a skilled professional to join our out-of-home care team as a Case Manager/Foster Care Coordinator. This role involves working with children, birth parents, foster carers, and support services to provide high-quality care and support.

">Key Responsibilities:">

  • Assessing the needs of children and families in care
  • Developing and implementing individualized plans to meet their needs
  • Providing regular supervision and support to foster carers
  • Collaborating with multidisciplinary teams to ensure effective service delivery
  • Documenting progress and outcomes in reports and case notes
  • Staying up-to-date with relevant legislation, policies, and procedures
">Required Skills and Qualifications:">

  • Bachelor's qualification(s) in social work, social welfare, psychology, or related fields
  • Knowledge, skills, and experience working with children and families
  • Demonstrated ability to write reports to a professional standard
  • Ability to work independently and as part of a team
  • Flexible to work some evenings and weekends
  • Current unrestricted driver's license
  • Fully vaccinated (2x minimum) against Covid-19
