About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team at Aged Care Homes. As a Registered Nurse, you will play a vital role in providing high-quality care to our residents and clients.
Key Responsibilities:
- Provide direct patient care, including administering medications, wound care, and clinical support.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the clinical care team to ensure continuity of care and effective communication.
- Conduct thorough assessments and evaluations to identify resident needs and develop strategies for improvement.
- Participate in handovers and multidisciplinary meetings to ensure seamless care transitions.
Requirements:
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum 2 years of experience working as a Registered Nurse in a residential aged care setting.
- Strong understanding of the Aged Care Funding Instrument (ACFI) and AN-ACC funding model.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Ability to work in a fast-paced environment and prioritize tasks to meet deadlines.
