
Client Advocate Specialist

About This Role:
You will work as a key contact for a group of clients receiving Home Care Packages. Your responsibilities include conducting comprehensive assessments, developing individualised care plans, and providing safe and person-centred care to help clients maximise their funding and achieve the best possible health outcomes.
Your role involves monitoring client needs, reviewing progress, and making timely adjustments when required. You will also ensure that funding is used effectively to enhance client wellbeing.
This is a varied and rewarding position where you will balance professional expertise with compassion, advocacy, and problem-solving to support our clients.
Key Requirements:
- Experience in case management, aged care, or a related field (typically 3+ years)
- Knowledge of the aged care sector, including quality standards and codes of conduct
- Strong communication and relationship-building skills
- A client-focused approach, with confidence in advocacy and decision-making
- Experience in budgeting and financial management
- Solid IT and documentation skills
- A current National Police Certificate (or ability to obtain) and driver's licence
