Care Coordinator

As a key member of our multidisciplinary team, you will play a vital role in promoting the mental health and wellbeing of adults accommodated in correctional centres.
Key Responsibilities:- Triage welfare support requests to improve mental health outcomes.
- Collaborate with other health professionals to deliver mental health treatment and therapeutic activities.
- Assist in coordinating brief interventions, therapeutic activities and mental health screening.
- Support the coordination of disability services and related activities.
- Respond to service requests from prisoners and other stakeholders.
- Refer individuals for further assessment and interventions, and coordinate group-based wellbeing programs.
- Certificate IV in Mental Health, Allied Health Assistance and/or Disability is highly desirable.
- Effective communication skills and ability to establish relationships with internal stakeholders and external service providers.
- Ability to monitor service delivery, report and analyse data.
- Able to maintain accurate records and perform administrative duties as required.
- Ensure security and confidentiality of sensitive information.
