Job Description

We are seeking a dedicated and compassionate professional to join our Aged Care team as a Care Coordinator. In this role, you will be responsible for designing and managing individual care plans that align with each client's personal goals, health needs, and lifestyle preferences.

About the Role:

  • You will conduct face-to-face assessments and update personalised care plans
  • You will ensure participants receive appropriate funding and support services
  • You will lead and support a team of Support Workers
  • You will promote independence, safety, and well-being

Requirements:

  • Solid understanding of Aged Care Quality Standards
  • Demonstrated experience managing aged care caseloads
  • Strong communication and people skills
  • Exceptional organisational and time-management abilities

What You Bring to the Role:

In order to excel in this role, you will need to possess a degree in Nursing, Social Work, or related field (or equivalent experience), minimum 3 years' experience in aged care or community care coordination, current Driver's Licence and insured vehicle, Valid First Aid & CPR Certificate, Cleared National Criminal History Check and WWCC (or willing to obtain).
