Care Relationship Specialist

We are seeking a dedicated and compassionate individual to join our team as a Care Coordinator.
The successful candidate will be responsible for building strong relationships with clients and their families, assessing client needs, conducting care assessments and planning, maintaining accurate records, and supporting clients to meet their goals.
About the role:
- Assessing and planning care needs
- Maintaining accurate records as per Aged Care Quality Standards and NDIS Practice Standards
- Supporting clients to meet their goals
- Supporting Home Care Workers
- Re-organising work priorities to meet the needs of a fast-paced environment
Required Skills and Qualifications:
- Sound understanding of Aged Care Quality Standards and NDIS Practice Standards
- Organisational, administrative and time management skills
- Registered Nurse or Enrolled Nurse (Desirable)
