This role plays a vital part in HammondCare's Mission in Action, delivering high-quality, person-centered clinical care that supports individuals to live well in their own home.
Job DescriptionThe Clinical Partner works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care.
- Clinical Partners are responsible for assessing clinical care needs, developing, and reviewing care plans to deliver evidence-based, best practice clinical care holistically.
- They work in partnership with clients and key stakeholders to understand their unique goals, needs, and preferences, ensuring they receive tailored support.
To succeed in this role, you will need:
- A commitment to engage and align with HammondCare's Christian Mission and Motivation.
- Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
- Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
- Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
- Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.
- Ability to work effectively in a multidisciplinary team and independently across multiple sites.
- A current driver's license and the ability to travel between HammondCare's sites and services.
- Excellent administrative and computer skills, including digital health records and care coordination tools.
