About the Role

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Mission Australia. As a key member of our Psychosocial Support Program, you will play a vital role in providing recovery-oriented support to individuals living with mental illness.

Key Responsibilities
  • Provide Specialist Advice and Guidance

Offer expert advice and guidance to staff to support clients throughout their service journey, ensuring seamless access to tailored care and support.

Develop Holistic Case Plans

Collaborate with Recovery Coordinators and Peer Advocates to develop comprehensive, trauma-informed case plans and strength-based recovery goals that cater to the unique needs of each client.

Build Effective Relationships

Establish and maintain positive relationships with a range of stakeholders, including clinical services, community organizations, community housing providers, and other relevant health professionals.

Work with Referral Agencies

Work closely with referral agencies to ensure they have a clear understanding of our service operations and can effectively refer clients to our program.

Participate in Professional Development

Engage in clinical supervision, peer development, and other opportunities for professional growth and development as required.

Requirements for Success
  • Relevant Qualifications and Experience

Hold a degree in Social Work, Social Sciences, Psychology, or a related field, with at least 2 years of relevant experience in the Allied Health sector.

Understanding of Social Determinants of Health

Have a deep understanding of the social determinants of health, trauma-informed care, family-sensitive practice, and lived experience and personal recovery frameworks.

Experience in Complex Client Support

Demonstrate experience in working with complex clients to develop collaborative care plans and provide recovery goal-directed care.

NDIS Experience

Have experience working with the National Disability Insurance Scheme (NDIS) and a strong understanding of its principles and practices.

Excellent Organizational Skills

Exhibit excellent organizational, time management, and problem-solving skills, with the ability to work creatively and safely with clients from diverse backgrounds.

Proficiency in Electronic Health Records

Be proficient in electronic health record systems and Microsoft Office suite.

Full Driver's License

Hold a full driver's license with no restrictions.
