We are seeking an experienced Hospital Pharmacist to join our dynamic team at Northern Beaches Hospital, located in Frenchs Forest, Sydney. As a Clinical Pharmacist, you will be responsible for providing a comprehensive range of pharmacy services to outpatients and inpatients to ensure the quality use of medicines within the hospital setting.
- Provide efficient clinical pharmacy services to inpatients or outpatients as part of the rotational roster.
- Conduct pharmaceutical admission and discharge reconciliations, monitor medication outcomes such as therapeutic drug monitoring, to ensure quality use of medicine is upheld.
- Work cooperatively and effectively within a multidisciplinary team and provide advice regarding patient care.
To be successful in this role, you will need:
- General registration with the Pharmacy Board of Australia (AHPRA), with no restrictions on practice.
- Minimum 1 year Pharmacist experience in an Australian-based Hospital.
- Australian Citizen or Permanent Resident with ability to work in Australia.
