About the Role:

As a dedicated Care Coordinator, you will be responsible for overseeing the delivery of exceptional care services to residents. Your key responsibilities will include managing and achieving agreed performance indicators, coordinating care services, and ensuring they are correctly implemented.

Key Responsibilities:

  • Manage and achieve agreed performance indicators
  • Coordinate care services and ensure they are correctly implemented
  • Manage human resources in a fair and equitable manner
  • Collaborate with care teams on care matters and issues related to service provision
  • Consult with residents and their significant others to design, monitor, review, and coordinate care plans following comprehensive care assessment
  • Support the onboarding of care staff, ensuring a positive employee experience
  • Monitor training and education of staff to ensure development of skills and compliance
  • Provide support and supervision to staff, ensuring delivery of their role
  • Rostering of staff and scheduling of resident care services to meet required care needs
  • Manage and review all care-related incidents
  • Ensure care meets quality standards, including disaster response planning
  • Participate in on-call rosters as required
  • Ensure appropriate handling of medications by care staff
  • Coordinate services with external clinical professionals
  • Maintain comprehensive confidential client files, consistent with legislative requirements
  • Ensure billings are correct
  • Adhere to policies and procedures to minimise risk of workplace injury/illness
  • Ensure incidents are immediately reported, and appropriate response is taken to situations of risk or potential risk to health, safety and wellbeing
  • Understand and adhere to fire and emergency management plans and procedures

Requirements:

  • Tertiary qualification in aged care, health, nursing, community services and/or equivalent relevant experience
  • 5+ years experience in HCP management and/or clinical role managing teams and budgets
  • Demonstrated knowledge of DVA Community Nursing Program, Home Care Packages, Aged Care Quality Standards and Client Rights and Responsibilities
  • Flexibility with hours when workload demands, across a 24/7 business
  • First Aid and CPR Certificate
  • Unrestricted Australian working rights
  • Current national criminal check
