Job Title: Disability Support Specialist

We are seeking a part-time Disability Support Specialist to provide services for individuals with complex support needs in Perth's northern suburbs.

About the Role:
  • Coordinate disability supports based on participant goals and funding
  • Refer and connect participants to local providers in the northern region
  • Monitor budgets and prepare reports for plan reviews
  • Facilitate stakeholder meetings and case discussions
  • Support participants through crisis or complex situations
  • Build strong working relationships across local networks
Requirements:
  • Minimum 3 years of Level 2 NDIS Support Coordination experience
  • Confident working independently and managing your own caseload
  • Strong knowledge of the NDIS framework and northern Perth service providers
  • Compassionate, culturally sensitive, and client-focused
  • Excellent verbal and written communication
  • Must have full working rights, valid NDIS Worker Screening & WWCC
  • Based in Perth's northern suburbs with a driver's licence and insured vehicle
