
NDIS Support Coordinator Job Opportunity

We are seeking a part-time Disability Support Specialist to provide services for individuals with complex support needs in Perth's northern suburbs.
About the Role:- Coordinate disability supports based on participant goals and funding
- Refer and connect participants to local providers in the northern region
- Monitor budgets and prepare reports for plan reviews
- Facilitate stakeholder meetings and case discussions
- Support participants through crisis or complex situations
- Build strong working relationships across local networks
- Minimum 3 years of Level 2 NDIS Support Coordination experience
- Confident working independently and managing your own caseload
- Strong knowledge of the NDIS framework and northern Perth service providers
- Compassionate, culturally sensitive, and client-focused
- Excellent verbal and written communication
- Must have full working rights, valid NDIS Worker Screening & WWCC
- Based in Perth's northern suburbs with a driver's licence and insured vehicle
