Senior Case Manager Position

Manage a case load of clients under approved funding. Maintain budgets, onboard growth of new clients and manage care requirements to maintain client independence.
Key Responsibilities
- Effective time management
- Understanding of aged care standards
- Liaising with stakeholders and internal networks
Required Skills and Qualifications
A minimum of 1 year's experience in case management is essential.
Relevant tertiary qualification in the community sector is required.
Demonstrated experience in the aged/community/disability sector, particularly working with clients with complex needs, is necessary.
A full driver's license and own reliable registered and insured vehicle are required.
A smartphone is also necessary for this role.
Knowledge of My Aged Care portal/assessment process is required.
Ability to work in a team environment is essential.
NDIS clearance and police clearance within the last 6 months are required.
