About the Role

The role involves managing a caseload of clients under government-approved funding. Maintaining client budgets, onboarding new clients, and managing care requirements to maintain client independence.

Requirements
  • Evidence of at least 1 year's experience in case management
  • Relevant tertiary qualification in the community sector
  • Demonstrated experience working with clients with complex needs
  • Full driver's license and own reliable vehicle
  • Smartphone
  • Knowledge of My Aged Care portal/assessment process
  • Ability to work in a team environment
  • Desire to join an organization committed to caring for others
  • NDIS clearance
  • Police clearance
