Overview

Responsibilities
  • Manage a case load of clients under CHSP/HCP approved funding
  • Maintain budgets and manage care requirements to support client independence
  • Onboard growth of new clients and coordinate with stakeholders
  • Time management
  • Understanding the Aged Care Standards
  • Liaise with stakeholders and internal networks
What you bring
  • Minimum 1 year experience in Case Management
  • Relevant tertiary qualification in the community sector
  • Experience in aged/community/disability sector, particularly with clients with complex needs
  • Full Driver's Licence and own reliable registered and insured vehicle
  • Smart Phone
  • Knowledge of My Aged Care portal/assessment process
  • Willing to work in a team environment
  • A desire to join a team committed to the care of others in a mission-based organisation
  • NDIS clearance (or willingness to obtain one)
  • Police clearance (within the last 6 months or willingness to obtain one)
