We are seeking an experienced and skilled Credentialed Diabetes Educator to join our multidisciplinary team. As a part-time educator, you will work 16 hours per fortnight to assist with parental leave cover and provide diabetes self-management education and support to patients in outpatients, inpatients, and group programs.
Key Responsibilities:- Provide high-quality diabetes education and support to patients, focusing on empowering them to manage their condition effectively.
- Collaborate closely with other healthcare professionals to achieve optimal health outcomes for patients.
- Follow evidence-based diabetes self-management principles and encourage patients to take an active role in their healthcare planning and decision making.
- Registered Nurse or Accredited Practising Dietitian (or appropriate allied health professional) with Credentialed Diabetes Educator status or working towards the same.
- Advanced clinical skills and experience in acute and/or community nursing or dietetics.
- A completed Graduate Certificate in Diabetes Education or equivalent qualification.
- Excellent communication and interpersonal skills, with the ability to liaise effectively across a multidisciplinary team.
