About the Role

MiCare is seeking a skilled Care Manager to join our team, responsible for delivering high-quality care services to elders in their homes.

The successful candidate will work within our In Home Support programs, providing care and services that enhance the quality of life of our clients. This role is pivotal in facilitating person-centred case management, client support, and astuteness in assessment, care planning, and review.

Key Responsibilities:

  • Undertake initial assessments, care planning, ongoing review, and monitoring of elders as outlined in MiCare's Home Care Policies and in accordance with the Commonwealth Government Aged Care guidelines and Charter of Aged Care Rights.
  • Liaise closely with internal and external stakeholders to ensure elders receive services appropriate to their needs and in accordance with program guidelines.
  • Initiate case and planning meetings as required, ensure solid documentation, and follow through on elder health, care, and service delivery.
  • Develop an individualised budget for elders with a Home Care Package, to ensure spending compliance within the Home Care Package Guidelines.
  • Responsible for organising referrals and permanent placement where required.
  • Provide support and advocacy for elders and carers as required.
  • Be actively involved with quality review for the programs ensuring your documentation and processes meet compliance.
  • Develop and maintain working relationships with relevant agencies and networks to promote MiCare.
  • Respond to phone and online enquiries, public education to potential clients.

About You

To be successful in this role, you will need to have demonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues. You should also have a clear understanding of the Aged Care Quality Standards and solid understanding of Commonwealth Government community aged care programs.

You will be a proactive, reliable, and excellent communicator with attention to detail, able to work with others and be a participative and supportive team member. Additionally, you will need to meet compliance requirements such as holding a current First Aid and CPR certificate, valid Victorian drivers licence with an insured and registered vehicle, and ability to provide evidence of current flu and COVID-19 vaccination.

Qualifications

  • Tertiary qualification in Allied Health, Nursing, or Social Work.

Estimated Salary: $43,500 - $47,000 per annum (approximately $21 - $22.50 per hour), plus $10,000 car allowance and 11.5% superannuation.
