We are seeking a highly skilled and compassionate Care Manager to join our team at MiCare, a non-profit organization dedicated to providing exceptional care services to individuals from diverse backgrounds.
As a Care Manager, you will be responsible for ensuring that the quality of life of our elderly clients is enhanced through personalized care and support. Your role will be pivotal in facilitating this by utilizing your expertise in person-centred case management, client support, and astuteness in assessment, care planning, and review.
You will undertake initial assessments, develop individualized care plans, and monitor the progress of our clients in accordance with MiCare's Home Care Policies and Commonwealth Government Aged Care guidelines.
Key Responsibilities:
- Undertake initial assessments and develop comprehensive care plans tailored to the unique needs of each client.
- Liaise closely with internal and external stakeholders to ensure seamless coordination of services and effective communication.
- Initiate case meetings, ensure thorough documentation, and follow up on client health, care, and service delivery.
- Develop and manage individualized budgets for clients with Home Care Packages, adhering to program guidelines.
- Organize referrals and permanent placements as required, ensuring the best possible outcomes for our clients.
- Provide advocacy and support for clients and their families, addressing complex family and social issues when necessary.
- Maintain accurate records and contribute to quality reviews, ensuring compliance with regulations and standards.
About You
To succeed in this role, you will possess a strong understanding of sound case management principles within the aged care sector, preferably with experience working with complex family and social issues.
A tertiary qualification in Allied Health, Nursing, or Social Work is essential, along with excellent communication skills, attention to detail, and the ability to work effectively in a team environment.
You must also have a current First Aid and CPR certificate, valid Victorian driver's license, and access to an insured and registered vehicle. Flu and COVID-19 vaccination certificates are also mandatory.
