Aged Care Specialist Homecare Coordinator

Bethanie, a leading not-for-profit aged care and retirement living provider in Western Australia, is seeking an experienced Case Manager Assessor to join its Homecare team in the South Metro region.
This permanent full-time position involves working 38 hours Monday to Friday with flexible working arrangements available. The successful candidate will be based at the Port Kennedy Homecare office, traveling between all Bethanie sites and services to ensure an active approach to meeting Homecare Service needs.
Key Responsibilities:- Oversee and coordinate the onboarding process of new Home Care Package clients for the assigned region.
- Develop ongoing relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for the client's health.
- Assist clients in identifying ongoing needs and goals to support them to remain in their own home.
- Ensure all client budgets are monitored and managed in line with approved package level and individual client requirements.
- Be an effective member of the multi-disciplinary team to ensure all decisions and referrals for clients deliver a high standard of holistic care.
- Maintain reports, information, and support as requested by the Regional Homecare Manager to a highly quality and in accordance with policy and procedures.
The ideal candidate will have current knowledge of the Aged Care Standards and Homecare Case Management. This role requires comfort with both the principles of case management and the ability to pivot and manage multiple priorities to achieve positive outcomes for new and existing recipients of Bethanie Homecare services.
We are looking for someone with a willingness to uphold the Mission, Vision, and Values of Bethanie. Key qualifications include experience in a homecare case management environment either with a clinical background or more than 3 years as a Case Manager in a similar organisation.
Salary and BenefitsWe offer a competitive salary of AU$70,000 - AU$90,000 per annum, plus travel allowance of $0.92c per km and banking, insurance, and retail discounts and benefits.
